Providing patients with timely access to their personal health information is a basic building block towards empowerment and self-management for those with chronic conditions. At the same time, social networking is becoming an important conduit for the dissemination of health information as healthcare professionals interact with patients on-line. As the dynamic of health information moves toward patient-centred approaches, the personal health record (PHR) is emerging as an alternative or adjunct to the electronic health record (EHR). This raises questions about the level of trust that will be accorded by physicians and other caregivers to PHR data that has been entered by the patient or unknown care providers, as well as questions about the extent to which healthcare organizations should share their data with patient-managed records. This panel will discuss these questions from the physician and hospital viewpoints.

However, at the same time, effective use of health information for secondary purposes needs to face the realities of data quality and appropriate governance structures to ensure long-term viability.
